POSITION SUMMARY The Hospital Maintenance Supervisor provides direct supervision to the Facilities Management maintenance and electrical staff including managing disciplinary actions as needed; performing quality management responsibilities; assists the Director of Facilities Management (Director) in financial management responsibilities; personnel development functions to maintain a productive work environment; performing other duties consistent with the job classification as assigned or requested by the Director. Oversees and monitors the maintenance and repair of all hospital buildings, electrical systems, and grounds, plans and schedules work to ensure adequate coverage and resources.  In coordination with the Facilities Engineers and management team, participates in the planning, design, and preparation of cost estimates for annual maintenance projects.  To ensure minimal impact to departments and functions, the Supervisor coordinates maintenance activities with appropriate hospital departments. 

POSITION SUMMARY The Clinical Pharmacy Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacy Specialists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. In these clinical roles, Clinical Pharmacy Specialists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacy Specialists serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Clinical Pharmacy Specialists provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy Students and pharmacy residents. If applicable, participation in the quality management program is expected with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve and evaluating the effectiveness of those interventions.  Responsibilities may include working to establish and maintain long- and short-term goals for the Quality Management Program; monitoring and documenting Quality Improvement Projects for progress in meeting QI goals; and providing guidance and education to staff on Quality Management priorities and projects.

POSITION SUMMARY In consultation with the Director of Emergency Services, the Lead Clinical Nurse (LCN) will implement work systems that assure the optimal use of all assets, staff and equipment.  The Lead Clinical Nurse evaluates the Nursing needs of patients, develops a plan of nursing care, initiates patient care assignments, implements and modifies them as needed.  In addition, provides or directs the provision of comprehensive Nursing care, delegation of duties to appropriate personnel, teaches and supervises their work as needed.  Participates in the evaluation of the quality of patient care.  May serve in the capacity of the Acting Director of Emergency Services in the absence of the Director.

POSITION SUMMARY Under the direction of the Public Health Director, Project Manager, and Lead Public Health Technician or local supervision, the Public Health Technician will provide public health interventions to include pandemic/epidemic preparedness; infectious disease case investigation/contact tracing; screening for infectious disease; vaccination outreach; community health work; community connecting & resource delivery; health education & coaching related to primary, secondary, and tertiary prevention; population health measurement; and public health data support. The incumbent will conduct case investigation/contact tracing in response to infectious disease(s). The incumbent will provide evidence-based intervention, education and public health communication to community members. The Public Health Technician will support community members in reaching optimal health without discrimination. The Public Health Technician will utilize patient tracking and data collection software and EHR to provide and document services provided. This is a CDC CHW CCR grant-funded position.

POSITION SUMMARY The Lead Quality Assurance Technician (QAT) is part of a highly functioning team that helps ensure consistent data input processes across the organization for public health data integrity. While following established protocols, responsibilities include: confirmation of appointments and providing upcoming patient appointment reminders; correcting data errors and missing data as it relates to public health services; identifying and carrying out public health service support duties. The Lead QAT will serve as a superuser for the Community Health Division for training, development and testing of the electronic health record, workflow development, and new program implementation. The Lead QAT will serve as the preceptor to their duties when onboarding new employees.  The incumbent will audit public health data, ensure accurate data input for purposes of public health data and revenue, such as vaccination, billable services, and other public health efforts as they arise. The Lead QAT will work closely with public health data analyst, clinical staff, and support staff across the organization to ensure consistent entry and processes that affect public health care and public health data. This is an American Rescue Plan Act-funded position. 

POSITION SUMMARY Call Center Representative is part of a highly functioning team who helps with operations of Tuba City Regional Health Care Corporation, which includes satellite clinics in LeChee, Bodaway/Gap, and Flagstaff. The Call Center Representative interacts with patients via telephone and perform a range of duties which includes, scheduling, gathering required information in preparation for the patient’s appointments. Providing effective customer service to those that need Health Care services. While following established clinic protocols, responsibilities include; educating patients of the care team and how they can improve their access to care; confirmation of appointments; answering patient questions to connect them to those who can help; identifying and carrying out clinical support duties associated with patient care services and administrative provider support. POSITION SUMMARY The majority of the workload is pharmacy supply management for the entire TCRHCC Pharmacy Department.  The primary knowledge and skills used will involve inventory management, procurement processes, management of automated and computerized systems and working closely with pharmacy staff and other TCRHCC staff to ensure adequate availability of medications throughout the hospital, clinics, and pharmacies.  The work product directly affects the health of inpatients and outpatients. All pharmacy service is accomplished in accordance with Federal and state law, Department of the Health and Human Services Regulations, IHS requirements, and Drug Enforcement Agency regulations.

POSITION SUMMARY This position is responsible for providing inpatient and outpatient medical nutrition therapy appropriate to medical diagnoses; providing diabetes self-management education; and using public health approaches to prevent and treat obesity, diabetes, and complications of diabetes. Incumbent also participates in weekend/holiday rotating on-call schedule to provide medical nutrition therapy for inpatients, with and without diabetes.

POSITION SUMMARY This position serves as coordinator of the following victim service grant programs: sexual assault, domestic violence, sexual assault nurse examiners, the sexual assault response team and advocacy. The incumbent will provide capacity building for all grant programs in collaboration with grant management, employees, members, advocacy agencies, law enforcement and community agencies.  The coordinator will abide by guidelines set forth by grant management and the grantor. The incumbent is responsible for the overall collection, retention of data, monitoring of performance improvement, and generating reports.  This position will facilitate and participate in the Sexual Assault Response Team (SART) initiatives and community outreach. The incumbent will act with independence in performance of duties, conducting periodic reviews and analysis of program operations, training and contingency plans. POSITION SUMMARY This position will provide services in the Cardiopulmonary Department as assigned.  The incumbent will work under the supervision of a Licensed Independent Practitioner (e.g. physician, dentist, and nurse practitioner).  Medical assistants perform a broad range of administrative and clinical duties to facilitate the work of the Licensed Independent Practitioner (LIP). Certified Medical Assistant will be responsible for preforming essential functions pertaining to Cardiopulmonary Department, gathering necessary vital signs from patients, prepare and send laboratory specimens for routine tests, sets up and cleans examination room, maintains supply stock in between exams, record data under direct supervision of the provider or LIP.  

POSITION SUMMARY Responsible for performing front desk receptionist, administrative and clerical duties for the Health Promotion Disease Prevention (HPDP) Department.  Duties include answering phones, taking messages, greeting patients, data entry, departmental file management, inventory/supply management, and department mail. Understands and follows the organization’s policies, procedures, and standards to include HIPAA, and Privacy Act. Incumbent will work with hospital Electronic Health Record (EHR) for scheduling, data entry, and patient registration for pediatric obesity intervention program the adult diabetes prevention program.  Incumbent needs to be an independent worker yet a team player, helping to meet the day-to-day needs of the HPDP Department and assist HPDP Program Director as needed.  This position is grant funded.

POSITION SUMMARY This position provides professional administrative support for the Chief Medical Officer (CMO) and/or designee of Clinical Services and reporting departments. Responsibilities include but are not all inclusive to assisting the CMO with special and ongoing projects to ensure that goals and objectives are being met to achieve maximum efficient outcomes, answering routine and non-routine correspondence with internal contacts of the organization, assembling highly confidential and sensitive information and dealing with a diverse group of important external callers and visitors. POSITION SUMMARY This position is located in the Surgery Department of the Tuba City Regional Health Care Corporation. Surgical specialties include: General/Trauma Surgery on adult and pediatric patients, and Urology. This position provides administrative and technical support for the outpatient clinics by promoting and maintaining an efficient functioning of the clinics.  Duties include entering and retrieving patient data from automated patient systems utilized by the hospital and developing and maintaining various patient registries. The incumbent initiates and carries out a variety of medical support duties associated with the care and treatment of patients. Additionally, collaborating with the General/Trauma Surgery specialty by coordinating with surgeons and mid-level providers, filing pre-operative evaluations, and maintaining post-operative follow-ups. Management of daily surgical office functions includes collaboration with Social Workers, Case Managers, Surgical Schedulers, Clinic Managers and Trauma Program Managers.

POSITION SUMMARY The Physical Therapist I/II / Advanced Physical Therapist I/II position serves to provide quality physical therapy services to a predominately Native American population for the treatment or prevention of physical disability or disease.  Responsible for management and implementation of Physical Therapy program; provision of physical therapy evaluation and treatment of inpatients (regular work week, and weekend rotation) and outpatients, ages ranging from birth through geriatric; provision of specialty services such as wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, assistive technology/adaptive equipment evaluation/treatment, Durable Medical Equipment Prosthetic Orthotic Supply Program; assumes direction and development of the physical therapy section to include continuing quality improvement, risk management and safety, physical therapy policies and procedures; Joint Commission preparation, staff education, reporting, staff meeting and in-service, and other duties.  Responsibilities also include that listed on the Medical Staff Credentialing Core Privileges form.

POSITION SUMMARY The purpose of this position is to provide a comprehensive environmental health services program within the TCRHCC service area.  This is an entry level position and requires a broad knowledge of biological sciences sufficient to understand the basic concepts, principles, methods, and techniques of environmental health and injury prevention as well as familiarity with epidemiological investigations.  The incumbent is responsible for implementing environmental health evaluation activities, analyzes results, identifies risk factors, compares results to existing standards, codes and regulations and makes recommendations to reduce risk factors.  Performs other environmental health duties as assigned.

POSITION SUMMARY The Career Development Coordinator is responsible for the oversight of the health disparities grant programs at the Tuba City Regional Health Care Corporation (TCRHCC). The Coordinator is responsible for the daily oversight for the following programs: Pharmacy Technician Trainee, Coding & Billing, Certified Medical Assistant, and Information Technology (entry level position).  The Coordinator will act as liaison between outside entities, management, and senior operational leaders in the development of talent pipelines of qualified candidates. The Coordinator is responsible for ensuring all recruitment activities are performed in accordance to the Navajo Preference in Employment Act (NPEA). This is a grant funded position.

POSITION SUMMARY Incumbent serves as Lead Social Worker/Registered Nurse (RN) Case Manager for TCRHCC and Sacred Peaks Healthcare Center (SPHC) and is responsible for direct supervision and supportive contact for the specific group of identified high-risk patients.  The Lead Social Worker/RN Case Manager is responsible for assisting the Director of Care Coordination, with management of the MSW and Case Managers with onboarding program, the day to day assignments, assisting in routine reporting, and supervisory duties. To the designing and managing a continuum of care focusing on empowering clients to achieve demonstrable outcomes and self-sufficiency.  The Lead Social Worker/RN Case Manager is responsible for assessment, service planning, and resource acquisition, monitoring progress and initiating and responding to emerging client needs.  The Lead Social Worker/RN Case Manager will link the client with Community Social Service providers, health care providers, substance abuse, and mental health providers to achieve specific goals.  The Lead Social Worker/RN Case Manager will have experience working with culturally diverse low-income populations having multiple barriers to self-sufficiency such as: illiteracy, welfare dependency, domestic violence, substance abuse, and mental health issues.  The Lead Social Worker/RN Case Manager will work with a multidisciplinary team within the service delivery setting.  The Lead Social Worker/RN Case Manager will be committed to Case Management Services and have some assignments with Purchased and Referred Care (PRC) formally known as Contract Health Services, providing assistance to the PRC Case Specialist.  The Lead Social Worker/RN Case Manager will be responsible to the Director of Case Care Coordination.

Ensures data/information is current with regulatory agency and best practice standards. POSITION SUMMARY The Radiologist interprets and evaluates extremely difficult diagnostic radiologic procedures and is often called upon to urgently evaluate many exams, including CTs, ultrasounds and teleradiographs.  Plus provides consultation to others to interpret radiologic findings and assist in determining the nature and the extent of diagnosis covering all body systems.  Manages patients through the continuum of hospital care; orders treatments, tests, and consults with specialty physicians and primary care physician where appropriate.

*PSYCHIATRIST   POSITION SUMMARY The purpose of this position is to serve as full time psychiatrist with assignments involving full range of cases, including the very difficult in their specialty, with very little or no technical guidance.  Incumbent is responsible for recognizing difficult-to-identify symptoms or signs; and is responsible for developing a full treatment regimen involving knowledge of new techniques or the use of prolonged or complicated procedures.  They are recognized as experts in their specialty area.     *PSYCHIATRIC NURSE PRACTITIONER   POSITION SUMMARY This position is responsible for overseeing the psychological service needs of individuals, families, groups, and the community at large.  Also entails conducting formal and informal assessments, conducting clinical interviews, and developing treatment plans for patients with complex mental or emotional problems.  Duties may include facilitating groups, conducting educational workshops, and participating in outreach programs.  Candidate must adhere to evidence-based practice guidelines, professional standards, and Tribal, State, and Federal legal requirements.  Work schedule consists of taking call on an assigned, rotating schedule that includes weekdays, weekends, nights and holidays.

The Nurse Practitioner of the Pre-Op clinic has overall supervision of the pre-operative clinic provided in the Surgery area, in addition to the flow of the pre-surgical testing and pre anesthesia clearance for patients to gain admittance to the operating room.  The Nurse practitioner of the Pre-op clinic will provide input and assist with major decision making processes in regards to cardiac, pulmonary, and overall general health clearance for optimizing the health of patients who will undergo future surgery.  They will play a major role in collaboration with the CRNA team and Internal Medicine department to decide if a patient is physically fit to undergo surgery and to decide if the patient’s laboratory markers deem them to be safe candidates for the administration of general anesthesia.  Through the development of algorithms and checklists by the CRNA and surgical/Internal medicine team the NP of the Pre-OP clinic will follow these flowsheets to ensure safety of the surgical patient.  This pre-op clearance process will allow EKG’s, labs, concerning physical disabilities, medication issues, and allergies to be pre-assessed prior to the day of surgery optimizing patient outcomes and preventing medical errors. The NP of Pre-op clinic will work along with the Chief CRNA to improve overall flow, function, safety and efficiency of the pre-operative and pre-anesthesia processes.
